It happens when someone gets subbed out just as they die. The bot will count that death as normal but give the player subbed in the same amount of deaths that the player had left before they were killed.

Ya, happens all the time. Back on gantz foxes was really good at subbing you as you died so it happen'd a lot. Freaked me out the first time to though.
