The word massive has always been associated with SubSpace & Continuum, hence "massively multiplayer". Basically when SubSpace first came out, it was the first game of its genre, which allowed online multiplayer with no defined player limits (a single arena can hold up to 100 players, and even more). Which still makes Continuum an MMO game.
