The version without the "not" appeared in the States in the 60s and is less common in other English speaking regions, but it is considered correct usage. The main difference is that the two phrases are stressed differently with the American usage emphasizing the word "care" and is said in a more sarcastic tone. As far as the meaning is concerned, yes, if you follow the actual literal meaning of the words it doesn't make sense, but in practice it's usage is like saying "big deal". Someone would call something a big deal even though the speaker really doesn't think it's a big deal at all. "I could care less" is used with the same tone and in the same situation.

I hate a few things, the chalkboard scratching shit, little kids who are so fucking loud and u ask them to stfu and they dont, when you're trying really hard to impress people with whatever you're doing, and they're licking balls to someone who u know doesn't deserve it, when u squish and rub cotton together so it makes that shitty ass FUCKING NOISE HOLY SHIT I CAN'T TAKE IT, i dont even wear like pure cotton socks or anything i can't take that noise/feeling
